EMPLOYEE ASPHYXIATED WHEN TRENCH CAVES IN
On October 18, 1989, Employee #1 and a coworker were working inside an unstable, twelve- feet-deep trench. The trench caved in, trapping Employee #1 and compressing his chest. Employee #1 died of asphyxia.
