Employee killed in fall from water tower

Employee #1 was welding antenna mounts (brackets) to the leg of a water tower for use by a cellular phone company in mobile phone transmissions. His only fall protection was a lanyard wrapped around the tower leg. This lanyard either came open or was not in use when Employee #1 fell approximately 80 feet to the ground. He was killed, sustaining head injuries.
