Investigation Summary
At approximately 1 p.m. on May 31, 1991, Employee #1 was working under a truck that had been raised by a hydraulic jack placed under the first of the truck's three axles. The truck engine was on and it was in gear. The truck came off the jack and ran forward approximately 7.5 feet until it struck other pieces of equipment. Employee #1 was killed when the truck ran over his head and chest. The employer had been using improper stands.
