Investigation Summary
At 8:13 a.m. on June 23, 2015, an employee was replacing the right hydraulic drive motor in a Case 60 XT skid steer loader. As the employee climbed through the front of the machine and underneath the raised operator's cab to access the engine compartment, the operator's cab closed down on him. The employee died as the result of mechanical asphyxiation. The "cab hold open latch" was not locked or tagged in the engage position to prevent the released of stored energy created by gravity.
