Employee Is Killed When Caught Between Operator And Engine C

At 8:13 a.m. on June 23, 2015, an employee was replacing the right hydraulic dri ve motor in a Case 60 XT skid steer loader. As the employee climbed through the front of the machine and underneath the raised operator's cab to access the eng ine compartment, the operator's cab closed down on him. The employee died as th e result of mechanical asphyxiation. The "cab hold open latch" was not locked or tagged in the engage position to prevent the released of stored energy created by gravity.
