Investigation Summary
Two employees were lifted 26 ft on a personnel platform that was improperly rigged to a hydraulic crane. The rigging detached from the crane hook and the platform dropped to the ground. Employee #1 suffered a fractured back. Employee #2 suffered a lacerated cheek. The primary cause of the accident was the use of two chains with open throated hooks to sling the platform to the crane hook.
