Employee dies of asphyxia after crashing skid steer

Employee #1 was using a Case 1845C Uni-Loader skid steer to grade and move dirt for a landscaping job at a new industrial building construction site. He backed the skidder over a 6 1/2 ft high concrete retaining wall, and it landed on its back before rolling over onto its left side. Employee #1 was thrown around the operator's compartment and knocked unconscious. His head and chest came to rest in a position that prevented him from breathing, and he died of asphyxia. Employee #1 was not wearing his seatbelt at the time of the accident, and this contributed directly to his being knocked unconscious.
