Employee killed in fall from roof

While other roofers were on the ground setting up, Employee #1 and two coworkers were on the roof of an apartment complex unloading equipment. At some point, the two coworkers left the roof while Employee #1 remained. He was wearing ppe, but was not tied off. Employee #1 fell 27 ft from the roof and died of head injuries.
