Employee killed when crushed by falling frame assembly

Employee #1 was attempting to replace a forklift fork that had fallen off a forklift mast. A farm equipment frame assembly that was standing on edge fell over, striking Employee #1 and crushing him against the forklift mast. Employee #1 sustained massive head injuries and died.
