Employee dies after hand caught between rollers

Employee #1 was the team operator on the third shift (11:00 p.m. to 7:00 a.m.), responsible for line operations and product monitoring. He usually operated tack nip machine #2 from inside a control room. At approximately 1:15 a.m. on September 13, 1996, the machine went down and needed to be manually restarted by three employees and then monitored. Employee #1 was near the tack nip machine where the product (blankets) was dumped from an elevated hopper and flattened by two 22 in. diameter steel mill rollers onto a conveyor belt. The product then traveled on a conveyor belt through two metal rollers: a 12 in. diameter steel roller and a 4 in. diameter steel fixed idler. Employee #1's hand became caught in the nip point of these two rollers, but he was able to free himself. Although the two coworkers on the line did not witness the accident, they realized when they saw him that Employee #1 was badly injured. They administered emergency first aid until the EMTs arrived. Employee #1 was taken to a local hospital and underwent corrective surgery on his hand. He died the next morning, September 14, 1996.
