Investigation Summary
Employees #1 through #3 were making repairs on the USS David Ray in dry dock #3 at the Long Beach Naval Shipyard. The rails on the 13 ft 8 in. high staging on the scaffold erected next to the ship had been removed so a fair water could be installed. An Ingersoll Rand forklift, serial #143726JF, and a chain fall, serial #H1558, were used to lift the fair water. Employee #1 had raised the chain fall to pull up the fair water, but the top hook of the chain fall hook was not secured. He was struck by the fair water and fell from the scaffold onto the forklift tire, and then head first to the dry dock deck. Employee #1 died later that morning. Employee #2 became caught between the scaffold railing and the fair water; he sustained mental trauma, bruises, contusions, and a strained back and right shoulder. Employee #3 had been trying to hold the fair water in place with his right hand when it broke loose, swinging him around and knocking him off the scaffold. He caught hold of one of the rails and suffered a strained right shoulder. None of the employees were using fall protection.
