EMPLOYEE #1 WAS PERFORMING MAINTENANCE UNDER AN ELECTRIC PALLET JACK WHICH WAS SUPPORTED BY TWO 35 INCH HIGH METAL SAW HORSES. THE JACK SUDDENLY FELL, STRIKING EMPLOYEE #1 IN THE HEAD AND CHEST. HE WAS KILLED. THE EMPLOYEE WORKED AT THE WHEELER FARMS, MACDOEL, CALIFORNIA.
