Investigation Summary
An employee had finished connecting test leads for dielectric testing of an Altec material-handling aerial lift. As he dismounted the back of the truck, he apparently lost his footing. (Witnesses saw him falling only after they heard him yell.) The employee fell backward 2.3 meters from the truck to the concrete floor. His left shoulder hit the floor first, and then his head whip lashed against the floor, with his body coming to rest on his left side. The employee died of a massive head injury.
