Investigation Summary

Investigation Nr: 202064242
Event: 06/15/2004
Employee Is Killed In Fall

Two employees were working installing metal siding on a power plant. The employees were using a JLG lift to access the work area. The lift consisted of an articulating boom with a basket (platform) installed. At lunch time the employees moved the basket to a permanent platform, installed at the power plant, to egress. They disconnected their safety lanyard from the basket and climbed over the guardrails. After lunch they returned to the platform. One employee climbed over the guardrails into the basket where he fastened his lanyard. The other employee handed tools and materials to the employee that was in the basket. When Employee #1 finished transferring the tools and equipment he attempted to climb over the guardrails into the basket. During Employee #1's attempt to get into the basket he fell approximately 82 ft to his death. The guardrails and basket were wet and slippery.
