Investigation Summary
On June 10, 2004, Employee #1 and his supervisor were unloading an enclosed bed truck that was loaded with electrodes. The electrodes were approximately 10-feet long and weighed about 4,500 lbs. During shipment the blocking and bracing, used for stabilizing the electrodes, was damaged. One of the electrodes, stacked on top of another electrode, was removed. As Employee #1 and his supervisor were removing the blocking and bracing, located between the two doubled stacks of electrodes, his supervisor stepped out of the truck in order to get the forklift. One of the top electrodes rolled off the stack and crushed Employee #1 against another electrode located on the floor. Employee #1's supervisor had only been in the title of supervisor for less than two months, starting on April 29, 2004. Additionally, he had not completed the supervisory accident prevention training, causing him to overlook and fail to recognize the hazardous condition present, resulting in the death of Employee #1.
