Investigation Summary

Investigation Nr: 920116
Event: 07/27/1992
Employee dies in fall from roof

A crew was nearing completion of the day's work. Employee #1 was scraping or cleaning the parapet wall that had been plastered that day. He was working with his back toward the end of the roof and was moving in that direction. A coworker had started off the roof to get a bucket of water to be used in the cleaning. A second coworker had gathered his tools and was leaving the work area for the day. When Employee #1 reached the end of the roof, his legs contacted the 18 in. high parapet, which is on three sides of the roof. He was killed when he fell backward over the parapet wall, 16 ft to the ground, landing headfirst.
