Employee killed when crushed in rotary screen printer

At approximately 3:40 p.m. on May 24, 1993, Employee #1 was performing a task under a rotary screen printer, model SDM 2020 Mega, manufactured by Stork MBK, while the machine was running. Employee #1's chest was crushed when his left arm and chest were caught and pulled between the conveyor blanket and a roller. Employee #1 was found dead under the machine approximately 40 minutes later. There were exposed moving parts under the machine and no lockout/tagout procedures were in place.
