Investigation Summary
Employees #1 and #2 went on a newly installed rubber roof approximately 20 ft from ground level to check the roof and bring down materials. The roof had a 2:12 pitch and the weather was sunny and cold. The sun had warmed one side of the roof, leaving it dry. The other shady side of the roof was covered with frost. When Employee #1 went onto the frosty side of the roof to retrieve a pail of roofing adhesive, he started to slide. Employee #2 went onto the frosty side to help Employee #1 and he too started to slide toward the roof's edge. Both employees slipped off the roof and fell 20 ft to the ground. Employee #1 fell onto a concrete slab and died of a head injury. Employee #2 fell onto a grassy area and sustained a back injury.
