Employee killed in 18 ft fall from unsecured ladder

Employee #1 placed a ladder on a lower level roof that extended to an upper level roof to install siding on a residential home. The bottom of the ladder slipped off the edge of the roof and Employee #1 fell approximately 18 ft to the ground. He died of massive chest trauma.
