Investigation Summary
At approximately 2:30 p.m. on September 6, 1995, a heavy equipment operator was transporting a heaped load of dirt in the bucket of a wheel loader machine through a 100 ft long by 17 ft wide alley. The machine was 11 ft 2 in. wide across the bucket and 10 ft 6 in. across the tires. The loader was traveling forward from south to north. When it was approximately 15 ft from the north end of the alley, it struck Employee #1, a mechanic, crushing him under the right wheels. Employee #1 was transported to the hospital, where he died.
