Investigation Summary
At approximately 3:00 p.m. on June 4, 2015, Employee #1, a journeyman electrician, was pulling an electrical wire through conduit while in a scissor lift near a junction box at the building's ceiling for outdoor lighting. There were three wires that ran through the junction box, numbered 9, 11 and 15. Wires numbers 9 and 15 were spliced to additional wires and supplying power to overhead lighting. Wire number 11 ran straight through the junction box to additional overhead lighting. Employee #1 had de-energized and locked out the circuit breaker powering wire number 15 in order to splice it to the wire being pulled, but did not de-energize wires number 9 and 11. The wire nut was either removed or bumped off and Employee #1's right hand to come in contact with wire number 9's energized 277 volt circuit. Employee #1 collapsed to the bottom of the uninsulated scissor lift and died from pulmonary congestion from the live electrical contact.
