Investigation Summary
A POWER LINE WORKER WAS WORKING FROM AN AERIAL LIFT, MOVING A TRANSFORMER UP TO A NEW LOCATION ON A UTILITY POLE. HE SECURED A STRAP HOIST JUST BELOW THE CROSSARM. HE SECURED THE OTHER END OF THE HOIST TO THE TRANSFORMER. DURING THE ENTIRE SETUP OPERATION, THE EMPLOYEE HAD BEEN WORKING CLOSE TO A 7200-VOLT OVERHEAD POWER LINE AND AN ENERGIZED FUSED DISCONNECT. AS THE EMPLOYEE JACKED THE STRAP HOIST, HIS RIGHT HAND CONTACTED AN ENERGIZED PART ON THE DISCONNECT. HE WAS ELECTROCUTED.
