Investigation Summary
Employee #1 and a coworker were preparing to unload a flatbed trailer containing four steel pipe racks and thirteen joints of pipe they had just delivered. One pipe rack at a time had been loaded at another site with a gin truck. The racks and pipes were chained to the flatbed trailer with four or five chains. Boomers were attached to the chains to tighten them. Two of the pipe racks rested on two of the chains. The employees were planning to loosen or remove the chains, attach a chain to the load, and drive the trailer from under the load. To reach the front bottom boomer, the last to be released, the employees had to lift the center bottom rack with a pry bar. When they pulled the pry bar out, the load shifted and two racks fell, knocking Employee #1 off the trailer. The racks landed on him and killed him. The coworker jumped clear and was not injured.
