EMPLOYEE DIES WHILE ATTEMPTING TO CONNECT COIL OF STEEL

Employee #1 died because he was struck by a falling material. He was attempting to pick up a coil of steel with an overhead crane operated by a pendant hand control. He inserted the inner diameter hole of the coil with the bottom carrying arm of the "C" hook. The coil apparently did not reach the back of the "C" hook and the coil slipped off striking the employee. He was pinned against another coil.
