Investigation Summary
Employee #1 was working on an elevated work platform that was approximately 5 ft to 6 ft 6 in. from the concrete floor. He was installing or repairing a dog chain used on a kill floor of the second legger area. Four or five coworkers were performing this task along the work platform. Employee #1 fell head first approximately 6 ft 6 in. to the concrete floor and was killed.
