Employee killed in fall from roof

Employee #1 was a member of a five-man crew that was laying 50 ft by 30 in. sections of metal roofing at a height of approximately 24 ft from roof eave to ground. It was his fourth day on the roof and his 30th day of employment; Employee #1 was exiting from the roof in order to go to lunch. The operator of a Lull all-wheel drive forklift with a work basket measuring 19 ft 10 in. long and 45 in. wide was positioned at the north of side of the building. The work basket was used for accessing and exiting the roof. The top guardrail was at eave height; employees stepped from the roof onto the top guardrail of the basket and jumped into it. No steps or ladder were available. Employee #1, who was the last to leave the roof, reportedly stepped on the guardrail as the work basket was being lowered. He stepped back onto the roof, lost his balance, and fell approximately 24 ft through the unfinished roof to the concrete floor of the building. He sustained head fractures and was killed.
