EMPLOYEE KILLED IN FALL FROM SCAFFOLD

Employee #1 was working on a metal bracket from scaffold erected around a 34 ft diameter steel tank being constructed approximately 40 ft above the ground. The platform was 37 in. wide but only two 2 in. by 10 in. boards were used that left a space of 5 or 6 in. on the outside of the planking and about 12 in. between the inside planking and the tank. This distance varied as much as 4 in. The guarding was by wire rope around the scaffold and the lower wire rope was 24.5 in. about the bracket. Employee #1 had a grinder that fell with him as he fell between the lower wire rope and the outside planking. The extension cord either caught him or he grabbed it and it pulled him upright just as he hit on his feet. His body continued falling and his head hit a metal grating behind him killing him.
