Investigation Summary
On May 4, 1988, Employee #1 was ascending an approximately 20 ft long fixed metal ladder. About halfway up he dropped some papers into an open area behind the ladder, where they landed on a false ceiling. Employee #1 tried to retrieve the scattered papers by holding onto the fixed ladder and leaning over toward them. He lost his balance or grip and went through the false ceiling. He fell over 10 ft onto a concrete stairway landing platform between some risers. Employee #1 died on July 14, 1988.
