Investigation Summary
Employee #1 was driving a fork truck to load material onto a trailer that was backed up to a dock elevated 4 ft above an adjacent driveway on the left. The employee was driving the fork truck in reverse and parallel to the edge of the dock when the left rear wheel went over the edge. The fork truck overturned, landed on the ground, and pinned the employee underneath. He suffered fatal head injuries.
