Investigation Summary
A crew of five carpenters, including Employee #1, had constructed the wood framework of a 2,400 sq ft storage building. The first story had 24 ft long trusses set at 10 ft centers and had a pitch of 3:12. The peak of the trusses was 11 ft and the ground to eave height was 8 ft. There were 2 by 4 in. purlins nailed to the trusses on 2 ft centers. While installing the first section of 3 ft by 12 ft long metal roof sheeting, Employee #1 fell 11 ft from a 2 by 4 in. purlin to the interior of the building, landing on the concrete floor. He was killed. Employee #1 was not using fall protection.
