Employee killed when head and chest are crushed in machine

At approximately 1:06 a.m. on October 25, 1991, Employee #1 reached his head, arm, and upper torso into a 7 1/2 in. opening to clear a paper jam. When the ream of paper cleared the machine's electric eye, the ream layboy table, on which the employee was resting his body, came up, crushing his head and chest. He was killed.
