Investigation Summary
An articulating boom with basket mounted on a fire truck was being tested by the new owner, the president of an electrical contracting company. On the third test trip of the day, Employee #1 and a coworker were riding in the basket as the owner operated the boom from the lower control station. According to witnesses, as the owner descended the booms, the upper boom began to move out, or up, instead of in or down. At the same time, the top of the personnel basket cocked in the direction of travel and paint began popping off the top center of the lower boom. The upper boom continued moving slowly past the vertical and down on the reverse side. Suddenly, with the basket and the two employees at a height of approximately 35 ft, the lower boom collapsed in the center. The basket slammed into the ground, killing Employee #1 and critically injuring the coworker. Apparently, metal fatigue was the cause of the accident.
