Employee killed when crushed by pipe racks

Employee #1 and a coworker were preparing to transport an oil and gas drilling rig to the next drill site a couple of miles away by means of a flatbed trailer. The coworker secured the hook of the winch truck to two pipe rack stands that had been previously secured together. Employee #1 engaged the power take-off (PTO) system on the winch in order to lift the two pipe racks onto the trailer. After the pipe racks had been loaded onto the trailer, Employee #1 left the cab of the truck to assist the coworker in securing the pipe racks. However, before the pipe racks could be secured they fell off and crushed and killed Employee #1. When the coworker went to the cab of the winch truck to lift the pipe racks off Employee #1, he discovered that the safety mechanism (DOGS) had not been engaged.
