Employee Is Killed by Falling Load

Employee #1 was working alone setting up a fragment impact test using 200-mm bullets. The employee used a forklift to move a large bundle of Celotex (4-ft by 4-ft by 12-ft, and estimated to weigh one ton.) The bundle had been placed upright on end against a metal barricade and it somehow fell over on top of Employee #1. Employee #1 was killed.
