Investigation Summary
At approximately 11:15 a.m. on November 26, 1990, Employee #1 was preparing to lay a length of sewer pipe in a trench that was approximately 25 ft long, 4 ft wide, and 7 to 8 ft deep. The trench walls were almost vertical and unshored. As the employee cleaned off the lateral line, the north bank and a section of the road curb caved in on him. The curb and soil pinned him against the opposite bank and almost covered him. The employee was pronounced dead at the scene from compression asphyxia.
