Investigation Summary
Employee #1 was cleaning hopper/applicators that contained the agrichemicals Lorsdan, Temik, Balan, or Dyfonate, or a combination of these. He was using water and his hands to break up compacted materials in the hopper. Because the rubber gloves that he was wearing had holes and his hands had numerous lacerations, the solution was absorbed into the body. Employee #1 died.
