Employee crushed under forklift after backing off dock

Employee #1 was operating a TCM 2 ton, type G forklift, model #FCG20N5, serial #3160166, manufactured by Toyo Umpanki Co., Ltd., Osaka, Japan. The employee was rolling, tying, and stacking carpet remnants. While using the forklift for stacking the remnants, the employee apparently backed off the dock. He became pinned between the forklift overhead protection and the ground, and suffered a crushed chest. EMTs and local police attempted to free him with the aid of jacks and employees from a nearby automotive shop. Employee #1 was pronounced dead at the scene. The weather conditions at the time of the accident were clear and hot. The employer had informed all the employees that only the foreman was allowed to operate the forklift.
