Employee killed when struck by falling pipe

At approximately 10:00 a.m. on October 23, 1991, Employee #1 was helping to remove water well piping from a bore, and guide the bottom of the pipe into a storage bin slide on the side of a truck, which was equipped with a Gardner-Denver GD 1500 portable drilling rig. The employee had started the pipe down the slide and had gotten off the back of the truck when the top of the pipe separated from the hook. The top of the pipe fell approximately 15 ft, striking Employee #1 on the left side of his head and neck. He was killed. The safety latch was not strong enough to prevent the pipe from separating from the hook.
