Investigation Summary
Employee #1 was on bridge #2. In crossing from the east side of the bridge to the south side to join two coworkers, he went directly under a load of plywood, 30 sheets to a bundle. The plywood started to slip and it fell through the nylon slings and onto Employee #1, killing him. Employee #1 was where he was supposed to be. The crane operator had blown his horn to warn all employees that a load was being flown on the deck of the bridge.
