Investigation Summary
At 9:30 a.m. on May 5, 2015, an employee was working on the ground as part of a crew that was trimming branches from a tree. The employee was moving cut limbs where a co-worker had scaled the tree and limbs were falling. The co-worker cut through an oak limb measuring approximately 5 inches in diameter and 30 feet in length and called out as it fell to the ground. The employee was underneath the branch as it fell and was struck in the head and back. A second co-worker witnessed the event and went to the employee who was lying on his back and was blinking heavily and unresponsive. Emergency Services were contacted and arrived on the scene shortly thereafter. The employee was transported to St. Anthony Medical Center in St. Louis where he was pronounced dead.
