Investigation Summary

Employee #1, an assistant house foreman, worked 7 1/2 hours of a scheduled 8 hour shift at a grain elevator. This was the employee's regularly assigned job. He left work early after complaining of tightness in his chest and breathing difficulty. The employee apparently drove directly from work to the local hospital. He collapsed outside the emergency room door. Efforts to resuscitate him were unsuccessful. He apparently died of a heart attack.
