Investigation Summary
EMPLOYEE #1 WAS SANDBLASTING A FREE STANDING STEEL PANEL. THE PANEL WAS 10 FEET LONG, 8 1/2 FEET WIDE AND 16 INCHES THICK. IT WAS COMPOSED OF TWO 8 INCH THICK PANELS JOINED BY RIGID STEEL STRAPS EXTENDIND BEYOND THE PANEL THICKNESS. THE PANEL WAS NOT SUPPORTED TO PREVENT IT FROM FALLING OVER. IT FELL ONTO EMPLOYEE #1 WHILE HE WAS SANDBLASTING. THE PROJECTING METAL STRAP PUNCTURED EMPLOYEE #1'S BACK, KILLING HIM.
