Investigation Summary
At approximately 2:00 p.m. on April 8, 2015, Employee #1 was performing a regularly assigned task of loading an office container onto a Superline lowboy trailer and working at a portable container delivery service facility. The trailer was positioned adjacent to another container. Employee #1 was found pinned between the trailer side rail and the adjacent container and suspended approximately 2.5 ft off the ground. There were no witnesses to incident. A customer had heard the trailer bed stop elevating and went to the area to investigate he initiated assistance. The employer arrived at the scene and moved the adjacent container to free Employee #1 prior to emergency response teams arriving. Emergency medical personnel arrived and transported him to a regional medical center, where he died.
