Investigation Summary
Employee #2 was operating a motorized hand fork truck to lift Employee #1 overhead. The mast on the fork truck had been removed. Employee #2 picked up a work platform, but did not secure it to the forks or any other part of the truck. While Employee #1 was 10 to 15 ft high removing pipe, he broke/cut into a line containing vinegar. The vinegar fell into Employee #2's eye as he was lowering and backing the fork truck up. The work platform fell from the forks and Employee #1 fell about 8 to10 feet. The work platform fell on top of him and cut his head. He was hospitalized.
