A CONCRETE MIXER TRUCK FELL INTO A TRENCH WHILE IT WAS DISCHARGING CONCRETE. THE TRUCK STRUCK EMPLOYEE #1, WHO WAS WORKING IN THE TRENCH, AND SEVERED HIS LEFT LEG.
