Investigation Summary
AT APPROXIMATELY 2 PM ON DECEMBER 5, 1986, EMPLOYEE #1, A POWER PLANT CONTROLS OPERATOR TRAINEE, WAS STANDING ON AN ELEVATED 2 INCH BY 12 INCH PLANK, SHAKING A 14 INCH PIPE TO CLEAR A BLOCKAGE. THE PLANK SLIPPED OFF ONE OF ITS SUPPORTS, ALLOWING EMPLOYEE #1 TO FALL APPROXIMATELY 8 FEET, 4 INCHES, AND STRIKE HIS HEAD ON THE TOP OF A CONCRETE RETAINING WALL. HE THEN FELL AN ADDITIONAL 5 FEET, 7 INCHES, STRIKING THE BACK OF HIS HEAD ON THE CONCRETE FLOOR. THE EMPLOYEE DIED ON DECEMBER 6, 1986, AT APPROXMATELY 8:20 PM, AT THE WAHIAWA GENERAL HOSPITAL.
