Investigation Summary
At approximately 8:15 a.m. on March 11, 1991, Employee #1 was about 20 ft up on the western end of a bridge abutment under construction. He was removing the whalers that held the forms together during concrete placement. The whalers were on 2 ft centers, enabling him to stand on one whaler and be tied off by a safety belt and lanyard to the whaler immediately above. He would then remove the whaler immediately above that. Employee #1 was experienced in this type of work, but for some reason he began to remove the whaler to which he was still tied off and fell approximately 20 ft to a concrete footer. He died of a fractured skull and cerebral trauma. The main causal factor appears to have been employee error.
