Employee injured in fall from roof of church

At approximately 2:15 p.m. on March 12, 1991, Employee #1 was working on the roof of a church that was under construction. Employee #1 fell off the roof, resulting in injuries to his head, left hand, and arm. The slope of the roof was 12:12 and the ground to eave height was measured at 17 feet 1 inch.
