ON NOVEMBER 15, 1990, EMPLOYEE #1 WAS HELPING AN OPERATOR BAG PEANUTS. EMPLOYEE #1 REACHED TO GRAB A BAG AND INADVERTENTLY INSERTED HIS RIGHT MIDDLE FINGER INTO THE MACHINE. THE MACHINE CLOSED AND AMPUTATED THE END OF HIS FINGER AT THE FIRST JOINT.
