Investigation Summary
Employee #1 and three other employees were pulling sheet piles. A personnel man-basket was used to raise an employee up to hook the sheet to the crane. The employee was then set back down on the ground and the man-basket was raised up above the sheet. After the sheet was pulled it was swung around to where it was to be stacked. Employee #1 and another employee were turning the sheets (working under the man-basket) when the basket broke. The crane operator yelled but Employee #1 was unable to get out of the basket's way. He was struck on top of the head by the falling man-basket and sustained severe head and back injuries. He later died from his injuries.
