Investigation Summary
Around 10:00 a.m. on September 2, 1988, Employee #1 was installing approximately 6 ft 6 in. long by 39 in. wide bridge decking pans. He had placed one pan and, while placing another, he struck the pan he was standing on and knocked it off the supporting beam. Employee #1 fell approximately 30 ft to the ground. He sustained bruises and scratches, for which he was hospitalized.
